Principal Purpose: Used by soldier in accordance with AR 600-8-8 when requesting a sponsor on his/her own behalf.
Routine Uses: To initiate assignment of a sponsor requested by the soldier.
Disclosure: Voluntary. Failure to provide personal information may result in a delay or error in processing of the request for sponsorship.
