    INFORMATION ON REPLACING AN EXPIRED, LOST OR STOLEN DEPENDENT ID CARD

    To replace a spouses ID card that is expired or about to expire, you’ll need the old ID card and a second form of identification such as a driver’s license, Social Security card or a birth certificate.

    To replace the expired ID card of a dependent child, stepchild, or ward, you’ll need the old ID card.  Due to the privacy act, the sponsor must be present, or you must have a valid Power of Attorney.  If the sponsor is deployed you can use their deployment orders in place of the power of attorney.

    To replace a dependent ID card for a name change, you’ll need the old ID card and two forms of identification showing the name change such as your Social Security card and driver’s license

    DOCUMENTS NEEDED FOR THE INITIAL ENROLLMENT OF A DEPENDENT

    For the initial enrollment of a new spouse, you’ll need the original or certified copy of the marriage certificate that was filed with the county.  Your spouse will also need to bring their original birth certificate, Social Security card and a valid photo ID.  If either you or your spouse were previously married you’ll also need all original divorce decrees, even if the divorce was prior to your military service.

    For initial enrollment of a child you’ll need the original birth certificate and Social Security card.

    To enroll a stepchild, you’ll need the original or certified copy of your marriage certificate, original birth certificate and social security card.

    To enroll a parent or parent in-law, you’ll need the dependency packet approved by DFAS, the parent’s original birth certificate, social security card, and a valid form of photo identification

    DOCUMENTS NEEDED TO REPLACE A COMMON ACCESS CARD THAT IS EXPIRED OR ABOUT TO EXPIRE

    To replace a CAC that is expired or about to expire, Soldiers and DoD civilians will need their old CAC and a second form of photo identification.  Contractors will also need a verification memo from the SID Office

    REPLACING A COMMON ACCESS CARD DUE TO PROMOTION

    To replace a CAC for a promotion you will need promotion orders, your old CAC and a second form of photo identification.   Individuals that have been demoted will need the reduction order and a second form of photo identification

    REPLACING A LOST OR STOLEN COMMON ACCESS CARD

    To replace a lost or stolen Common Access Card, all Soldiers E1 through E5, contractors, and civilians GS-10 and below will need a Fort Drum Form 506E signed by an E6 and above or by GS-11 and above.  All customers will need to produce a second form of photo identification

    ENROLLMENT OF COLLEGE STUDENTS AGES 21 TO 23 

     If your dependent is 21 to 23 years old and attending college full time you’ll need a recent letter from the College Registrar stating that your child is enrolled and attending full time, with an estimated graduation date.  Letters of acceptance are not valid, the letter must indicate full-time enrollment.  Your student will also need their old dependent ID card and a second form of photo identification
